This is an initial application for a corporation/partnership/LLC broker license. This form must be mailed to the LREC office along with any corresponding fees.

Complete this form if you are applying for or renewing an active license AND purchasing errors and omissions insurance independently in lieu of the LREC group policy.

This is an initial application for a branch office, which includes any office established for conducting any real estate license activity at an address other than the registered address of the broker. This form must be mailed to the LREC office along with any corresponding fees.

This is a sample corporate resolution. Broker Company applicants can choose to use this sample or create their own corporate resolution.

Use this form to terminate the supervision of a salesperson or associate broker. This form can be submitted by mail or email.

This form is used to transfer a license to a new broker. For a current associate broker wanting to become an independent broker, use this form and place your individual name and license number in Part I and Part II. No company info goes on the form to become an independent broker. You must also submit the Termination of Supervision to be removed from the current supervisor. This form must be mailed to the LREC office along with any corresponding fees.
