When conducting real estate business in the state of Louisiana you are required to use your Commission-issued license number. If you do not know your license number, you can find it using our Verify a License tool.
The following license maintenance forms are available for download. Adobe Acrobat Reader must be installed to view them. Please note: Fill-in forms cannot be saved using Adobe Acrobat Reader only. To maintain a record, please print prior to submission.
