Transfer to Inactive Status

Please note that if your intent is to have an inactive license for the upcoming year you should not use this form. Instead, you should follow the instructions on your renewal form to renew and transfer to the inactive status.

Current licensees in the active status who wish to immediately transfer to the inactive status must use the Transfer License to Inactive Status.
