License Maintenance Forms
The following forms are available for download. Adobe Acrobat Reader must be installed to view them. Please note: Fill-in forms cannot be saved using Adobe Acrobat Reader only. To maintain a record, please print prior to submission. Handwritten forms must be completed in blue or black ink only.
Change of Address
The paper form above can be used to update your address, phone number, and email address. You can also update this information with no paper processing delay by logging in to your MyLREC Portal.
Name Change Affidavit
Use this form to notify the LREC of an official name change.
Nickname Request Form
Use this form to notify the LREC of a nickname.
Company Name Change Form
Use this form to notify the LREC of an official company name change.
Initial or renewal licensees electing to obtain equivalent errors and omissions insurance coverage in lieu of the group policy shall submit the above Independent Errors and Omissions Insurance form before a license will be issued.
License/Certificate Verification Request Form
Use this form to request a license/certificate verification.
License Transfer Forms
Transfer from Inactive to Active
You must contact the LREC for a file review to begin this process.
Termination of Supervision
This form is used to terminate the supervision of a salesperson or associate broker. It is a fill-in form that can be completed online, printed, and submitted via mail.
Transfer License to New Broker
This form is used to transfer a license to a new broker. It is a fill-in form that can be completed online, printed, and submitted with payment via mail. For a current associate broker wanting to become an independent broker, use this form and place your individual name and license number in Part I and Part II. No company info goes on the form to become an independent broker. You must also submit the Termination of Supervision to be removed from the current supervisor.
**The paper forms above can be used to terminate and transfer license supervision. You can avoid paper processing delays by logging in to your MyLREC Portal to complete the terminate/transfer process. Transfer License to Inactive Status
This form is used to transfer a license to inactive status. It is a fill-in form that can be completed online, printed, and submitted with payment via mail.
Change of Designated Qualifying Broker for Corporation, Partnership, or L.L.C.
This form cannot be used to transfer a licensee from one broker to another. Use this form only to change the designated qualifying broker of a Corporation, Partnership, or L.L.C. This is a fill-in form that can be completed online, printed, and submitted via email.
