Fill-in forms cannot be saved using Adobe Acrobat Reader only. To maintain a record, print before exiting the document, or use the “save as” command to save it to your document files.

If the printed document fails to print in its entirety, check your printer settings. Failure to submit a complete application will result in processing delays. Use black or blue ink only to complete your application by hand.

Broker License Application “Part A”
This form is Part 1 of 2 of the initial application for a real estate broker license. It is designed for use by resident, non-resident and reciprocal broker license applicants.

Salesperson and Associate Broker “Part B” (Supervision Affidavit)
This form is Part 2 of 2 of the initial application for a salesperson or associate broker license.

Request to Issue in the Inactive License Status
If you do not plan on practicing real estate immediately and/or do not have a supervising broker, you may have your license issued in the Inactive status.

Termination of Supervision
This form is used to terminate the supervision of a salesperson or associate broker. It is a fill-in form that can be completed online, printed, and submitted via mail.

Initial Branch Office License Application
This is an initial application for a branch office, which includes any office established for conducting any real estate license activity at an address other than the registered address of the broker.
